This website is operated by Teacup Dementia Therapy Ltd. Throughout the site, the terms “we”, “us” and “our” refer to Teacup Dementia Therapy Ltd. Teacup Dementia Therapy Ltd offers this website, including all information, tools and services available from this site to you, the user, conditioned upon your acceptance of all terms, conditions, policies and notices stated here.
By visiting our site and/ or purchasing something from us, you engage in our “Service” and agree to be bound by the following terms and conditions (“Terms and Conditions”, “Terms”), including those additional terms and conditions and policies referenced herein and/or available by hyperlink. These Terms and Conditions apply to all users of the site, including without limitation users who are browsers, vendors, customers, merchants, and/ or contributors of content.
Please read these Terms and Conditions carefully before accessing or using our website. By accessing or using any part of the site, you agree to be bound by these Terms and Conditions. We reserve the right to update, change or replace any part of these Terms and Conditions by posting updates and/or changes to our website. It is your responsibility to check this page periodically for changes. Your continued use of or access to the website following the posting of any changes constitutes acceptance of those changes.

1. Consent

Engagement in the Teacup LifeStory process includes the client (the focus of the LifeStory Book) and a family member or carer. Where appropriate both parties will need to understand that involvement in the Teacup LifeStory process will require consents relating to:
Engagement in the process of therapeutic LifeStory intervention,
storage of data,
recording of LifeStory sessions,
provision of and storage of photographs,
involvement in evaluation of the process
Consents will be discussed fully and with both the client and their carer/family member during the Teacup Introductory Session with their assigned Memory Guide. A written summary of consents will be provided following the completion of this session. Consent can be withdrawn at any time during the Teacup LifeStory process however this may have some baring on our ability to provide a satisfactory Teacup LifeStory service. Where a person with a registered Power of Attorney (PoA) arrangement is consenting on behalf of a person with dementia a copy of the registered PoA documentation may be requested to be shown during the Teacup LifeStory process. Consent to engage in the process will still be sought from the person with dementia throughout the process.
In cases where a person with dementia’s own funds are being used to pay for the service by someone with PoA, a copy of the registered document must be shown either before or during the Teacup Introductory Session.

2. Confidentiality and Duty of Care
Teacup will comply with all confidentiality rules and guidelines outlined by The Data Protection Act 2018 and by their individual professional bodies (Royal College of Occupational Therapists/Nursing and Midwifery Council) except:
where disclosure is necessary to safeguard the individual, or others, or is in the public interest
where there is a legal duty to do so, for example a court order
Engagement in the LifeStory process is designed to be enjoyable, fulfilling and fun, however it can involve discussion about all areas of a person’s life, if during this process the Memory Guide has concerns regarding the safety or welfare of the Teacup client they are duty bound to address these using their profession specific Code of Ethics and Professional Conduct. It remains the responsibility of the client and/or their carer or family member to ensure the Teacup Memory Guide is informed of any areas the client does not wish to discuss as a part of the LifeStory process.

3. Data storage and privacy policy

We currently collect and process the following information:
Personal identifiers, contacts and characteristics (for example, name, address, and contact details – telephone number and email address)
Relevant health information including issues with sight and hearing
GP surgery details
Next of kin details
Telecare provider details
The story of your life so far
Recordings of LifeStory sessions
Photographs
Payment details

Most of the personal information we process is provided to us directly by you for one of the following reasons:
To receive updates, offers and information about Teacup services
To engage in the Teacup LifeStory process
To create your LifeStory book
We also receive personal information indirectly, from the following sources in the following scenarios:
A family member or carer may provide us with information about your life and photographs of you.
We use the information that you have given us in order to
Engage you in the Therapeutic LifeStory process
Create a hard copy LifeStory book

We may share this information with our book producers – currently Photobox. We may share this information with our partner organisation TanDEM care if you request we make a referral on your behalf for their services.

Your information is securely stored on Teacup password protected computers and a copy of your LifeStory book is stored on our book producer’s storage system (currently Photobox).
We keep your LifeStory book content and contact information for 3 years in order to reprint should you request further copies. We will then dispose your information by deleting from our computer systems.
The recordings of your Teacup therapeutic LifeStory sessions will be deleted from Teacup secure computer systems once you have received your hardcopy book.

By agreeing to these Terms and Conditions you agree not to record the Teacup therapeutic LifeStory sessions or any online interactions without our consent. If you would like to request a copy of our recordings from these sessions you can do so by emailing [email protected] We will hold these recording until you have received a hard copy of your LifeStory book.

4. Fees, payment terms, refund and cancellations

Fees paid when you sign up to the Teacup LifeStory process include 4-6 therapeutic LifeStory sessions, creation of one LifeStory book, postage and packaging – in accordance with the level of Teacup service you have purchased.

Full payment must be received prior to the Introductory Session.

Teacup operates a 14 day cooling off period. If you change your mind about your purchase within 14 days of purchase you will be entitled to a full refund. If you choose to start the process and commence with the Introductory Session before the end of the 14 day cooling off period you waive the right to the 14 day cooling off period.

The Teacup Introductory Session is an opportunity to meet your Memory Guide, test the technology and ask any questions about the process. We understand that your commitment to engage in the Teacup LifeStory process may change following our initial Introductory Session, if you change your mind following this session you will be entitled to a full refund.

Following a commitment to engage in the Teacup therapeutic process after the Introductory Session, the client and/or carer/family member will not be entitled to a refund if they terminate their involvement in the process. Accepting an invite to the first Therapeutic LifeStory session is seen as a commitment to engage in the Teacup process.

The Teacup LifeStory process is low risk and designed to be enjoyable, beneficial and meaningful for all involved, however if during the process the Memory Guide believes the process is causing harm to the client he/she can choose to end the Teacup process, in which case a refund for any uncompleted sessions will be provided.

During the Teacup LifeStory process there will be opportunities to review the level of service purchased and amend payment accordingly if the client wishes to do so. There will also be an opportunity to purchase further copies of your LifeStory book. No additional fees will be incurred without full discussion with all involved parties (Memory Guide, Client and family member/carer), and written confirmation of proposed additional fees will also be provided prior to any additional fees being incurred.

Before a LifeStory book is printed the client and family member/carer will receive a copy of the proposed text and corresponding photographs. Amendments to the text and photographs can be made at this time and returned to their Memory Guide as the FINAL copy. No changes can be made once the client/carer has returned the FINAL copy. Any mistakes or inaccuracies not highlighted in this FINAL copy will go to print and the client and family member/carer must accept responsibility for them; in such circumstances no refund will be provided. If however the final printed text differs from that of the amended FINAL copy received from the client, a replacement copy will be provided at no extra cost once necessary changes have been confirmed by the client.

5. Compliments and complaints

We value feedback to help us maintain our high-quality service. 6. Engagement in the Teacup process

The teacup LifeStory process involves the client and carer/family member spending some time outside of therapeutic sessions to fill in questionnaires, find photographs and upload photographs. By agreeing to the terms and conditions you agree to engage in this process. If you are unable to provide the required information for the completion of a LifeStory book then the process cannot be completed, under these circumstances a refund will not be provided.

Teacup will not accept responsibility for any photographs or information sent using the post. We ask that all photographs and materials for use in the LifeStory process are sent electronically. It is your responsibility to ensure you are using secure and accurate email/online facilities to provide such information.

Online Store Terms

By agreeing to these Terms and Conditions, you represent that you are at least the age of majority in your state or province of residence, or that you are the age of majority in your state or province of residence and you have given us your consent to allow any of your minor dependents to use this site.
